Thank you for applying for Fertile Action's financial aid program. Although the only criteria for our program is your cancer diagnosis verified by your oncologist's e-signature, we've compiled an application that gathers the kind of information that will help us make a difference for you and for others like you. Most of the information will only take you a few minutes to fill out. Once we process your application, an email is sent to your oncologist, the fertility clinic of your choice and any other providers you need to protect your fertility. Our streamlined approach only takes minutes to approve you and alert your providers ensuring you get access to fertility preservation services as quickly as possible.
